“Sublaminar fixation versus hooks and pedicle screws in scoliosis surgery for Marfan syndrome ”

In patients with Marfan syndrome (MFS), surgical correction of spinal deformities with hooks and / or pedicle screws results in a higher complication rate than in patients with adolescent idiopathic scoliosis. Therefore, sublaminar instrumentation is often an option of last resort. This study aims to evaluate the ability of sublaminar fixation to achieve three-dimensional correction of scoliosis and stabilization of the spine compared to hook and / or pedicle screw systems. The degree of correction with respect to the preoperative state was satisfactory with both approaches. The data from this study suggest that correction of scoliosis with sublaminar fixation is not inferior to treatment with hooks and / or pedicle screws.

In the following article, the authors present the possible application of sublaminar bands in oncological vertebral surgery, illustrating different cases. The different usage indications discussed can be divided into three categories:
• “Rescue” procedure, in case there is no possibility of inserting pedicle screws
• Additional tool for the reduction of kyphosis in pathological vertebral fractures
• Anchoring of bone transplants

The reduction and posterior fusion of spondylolisthesis present a few recognized and accepted procedural difficulties based on its severity. The Jazz ™ Band is an innovative system designed primarily for posterior fixation of the spine; however, its use can be applied to a wide range of spinal conditions. Its advantages include one size fits all spinal levels; the connector is designed for multiple bonding rod diameters and reduced comorbidity compared to other surgical methods. We present the case of a 26-year-old woman with a slip angle of the L5 / S1 spondylolisthesis of 117.4 °. A 25 ° improvement in the sliding angle of the spondylolisthesis was obtained with the use of the Jazz ™ Band. The Jazz ™ Band demonstrates excellent short-term results; to determine the clinical results and the effectiveness of the Jazz ™ Band system, it is necessary to perform long-term analyzes and results.

Hypocyphosis is currently observed in thoracic idiopathic scoliosis. The use of sublaminar bands allows a good restoration of the sagittal balance of the spine. The purpose of the study was to provide a medium-term radiographic analysis of patients suffering from adolescent idiopathic scoliosis with preoperative hypocyphosis, treated with posterior arthrodesis with sublaminar bands. This study concludes that the correction performed with sublaminar bands is effective for both the frontal and sagittal planes. Furthermore, the restoration of normal thoracic kyphosis is followed by an adaptation of the adjacent curvatures, with an improvement in the cervical and lumbar lordosis.
